- Firefighters
- Civilians and military personnel needing occupational health examinations

Return to Fly
If you are seen outside of the Flight Medicine Clinic, you may be placed on Duty Not Including Flying status.
To be cleared for Return to Fly, follow one of these steps:
- Send a secure message to the Altus FOMC via the MHS GENESIS Patient Portal
- Call the clinic at 580-481-5230 for further instructions
Initial Flying Class Exams
Before scheduling your Initial IFC exam, complete the following:
1. Submit a request via ASIMS
- Navigate to the Medical Clearance tab and click “Start a New Medical Clearance."
2. Complete the Medical History Form (DD 2807)
3. Wait 3-5 days for a Flight Medicine technician to contact you about your request.
Periodic Health Assessments
To initiate your PHA:
- Click the PHAQ link in the PHA column of your IMR Page (If you’re not due for a PHA, you won’t be able to start a new PHAQ unless unlocked by the MTF)
- Once your PHAQ is complete, send a secure message or call Flight Medicine at 580-481-5230 to schedule your Annual PHA/Flight Physical.
- This appointment will also fulfill the Annual Mental Assessment requirement.

To renew a waiver, copy and paste the questions below with your typed responses into a new secure message via the MHS GENESIS Patient Portal:
