Base Operational Medicine Clinic

The Base Operational Medicine Clinic is the hub for medical readiness at Altus AFB, serving active duty service members only. The BOMC manages all medical readiness programs, evaluations, and clearance to ensure Airmen are fit for duty, deployment, and career transitions.  

Services Provided 

Medical Readiness Programs 

BOMC oversees the following programs: 

  • Periodic Health Assessments 

  • Mental Health Assessments 

  • Deployment-Related Health Assessments 

  • Separation Health Physical Examinations 

Additionally, BOMC handles medical clearance processing for: 

  • Retraining and reclassification 

  • Overseas PCS assignments 

  • Palace Chase/Palace Front applications 

  • Professional Military Education and Command Selection Boards 

BOMC also includes the Medical Standards Management Element, which manages: 

  • Medical Profiles 

  • Airman Medical Readiness Optimization Board (AMRO Board; formerly DAWG) 

  • Medical Evaluation Board Process

Your Individual Medical Readiness Action List in ASIMS will display overdue medical requirements.

Medical Readiness Requirements 

Periodic Health Assessment 

  1. Initiate your PHA by clicking the PHAQ link on your IMR Page.  

  1. Schedule your appointment: 

  • Flying personnel: Message Altus Base Operational Medicine Clinic via MHS GENESIS Patient Portal or call the Warfighter Clinic at 580-481-5230.  

  • Air Force Reserve/Air National Guard (including IMA): if in-person PHA is required, Message Altus Flight Medicine Base Operational Medicine Clinic via MHS GENESIS Patient Portal or call the Warfighter Clinic Flight Medicine at 580-481-5230. 

  • First-time PHA in the Air Force? You must schedule an in-person PHA. Message Altus Flight Medicine Base Operational Medicine Clinic via MHS GENESIS Patient Portal or call the Warfighter Clinic Flight Medicine at 580-481-5230. 

  • All others: Contact BOMC via MHA GENESIS Patient Portal or call 580-481-5230 to schedule your MHA. Once completed, no further action is required.  

Deployment Health Assessments 

If you’re tasked for a deployment or have recently returned, you must complete a Deployment-Related Health Assessment: 

  1. Complete the online DRHA questionnaire.  

  1. Schedule a DHA appointment via MHS GENESIS Patient Portal or call BOMC at 580-481-5230.  

  • DHA 1&2: In-person appointments 

  • DHA 3,4, & 5: Over-the-phone appointments 

Separation Health Physical Exam 

This is required for separation or retiring service members. The process can start 179 days before final out date.  

Step to Complete SHPE: 

  1. Log into ASIMS , click the SHPE tab, complete the SHPE Questionnaire and MHA.  

  1. Secure message BOMC via MHS GENESIS Patient Portal or call 580-481-5790 to schedule.  

  • If you’re planning to see the VA, do not complete the SHPE MHA, contact BOMC for further instructions.  

  1. Schedule additional exams: 

  • Hearing exam (Public Health) must be completed before SHPE appointment.  

  • Final dental exam (Altus Dental via MHS GENESIS Patient Portal or call 580-481-5262)

Medical Clearance Processing 

How to Request Medical Clearance 

  1. Log into ASIMS, submit a new Medical Clearance Request, and complete the required medical questionnaire.  

  1. Secure message BOMC via MHS GENESIS Patient Portal with completed documents.  

IMPORTANT: 

If a clearance request isn’t completed within 72 hours, it will be deleted, and you will need to resubmit it.  

You can check the status of your clearance request in ASIMS.  

Types of Medical Clearance 

  1. Retraining/Reclassification 

  • Secure message BOMC via MHS GENEIS Patient Portal with your retraining myPers response email.  

  • Processing time: 3-7 duty days.  

  • Flying AFSCs require 10-12 weeks (contact Flight Medicine for details). 

  • If you wear contact lenses, do not wear them for 30 days prior to a Flight Physical.  

  1. Overseas PCS Clearance (Active Duty Only) 

  • Attach the following in MHS GENESIS Patient Portal secure message to BOMC: 

    1. Medical Clearance Memo 

    2.  Report No Later Than Date 

    3. Projected Permanent Change of Station 

    4. Estimated Departure Date 

  • Your request will remain pending until your medical clearance memo is received.  

  1. Palace Chase/Palace Front AF Form 422A Request 

  • Complete the medical questionnaire and provide a tentative separation date.  

  • SHPE will be completed after AF Form 422A request is approved. 

  • Submit all documentation via MHS GENESIS Patient Portal.  

  1. AFIT Assignment (Air Force Institute of Technology) 

  • Secure message BOMC via MHS GENESIS Patient Portal

    • Assignment Notification RIP 

    • Medical Clearance Memorandum 

  • Processing time: 3-7 duty days (Download completed AF Form 422A from IMR) 

Additional Medical Readiness Services 

Dental Exams 

If schedule your annual dental appointment, call Altus Dental at 580-481-5262. If you are in the Air Force Reserve/Air National Guard outside the Altus local area, call us and we will coordinate your care.  

Labs (HIV Testing) 

HIV labs are walk-in services, no prior lab order required. However, lab orders must be entered into the system before the patient arrives.  

Immunizations 

All vaccine appointments: Call 580-481-5235 

TB Tests: 

  • Schedule for the afternoon and return 48-72 hours after placement.  

  • No TB tests on Thursdays or before long weekends.  

Travel vaccines: Visit Public Health, contact them via MHS GENESIS Patient Portal, or call 580-481-5488
