Base Operational Medicine Clinic
The Base Operational Medicine Clinic is the hub for medical readiness at Altus AFB, serving active duty service members only. The BOMC manages all medical readiness programs, evaluations, and clearance to ensure Airmen are fit for duty, deployment, and career transitions.

Periodic Health Assessment

Schedule your appointment:

Deployment Health Assessments
If you’re tasked for a deployment or have recently returned, you must complete a Deployment-Related Health Assessment:

Complete the online DRHA questionnaire.

Separation Health Physical Exam
This is required for separation or retiring service members. The process can start 179 days before final out date.
Step to Complete SHPE:

Log into ASIMS , click the SHPE tab, complete the SHPE Questionnaire and MHA.

Medical Clearance Processing
How to Request Medical Clearance

Log into ASIMS, submit a new Medical Clearance Request, and complete the required medical questionnaire.

Secure message BOMC via MHS GENESIS Patient Portal with completed documents.
IMPORTANT:
If a clearance request isn’t completed within 72 hours, it will be deleted, and you will need to resubmit it.
You can check the status of your clearance request in ASIMS.
Types of Medical Clearance

Retraining/Reclassification

Processing time: 3-7 duty days.

Flying AFSCs require 10-12 weeks (contact Flight Medicine for details).

If you wear contact lenses, do not wear them for 30 days prior to a Flight Physical.

Overseas PCS Clearance (Active Duty Only)

Palace Chase/Palace Front AF Form 422A Request

Complete the medical questionnaire and provide a tentative separation date.

SHPE will be completed after AF Form 422A request is approved.

Additional Medical Readiness Services
Dental Exams
If schedule your annual dental appointment, call Altus Dental at 580-481-5262. If you are in the Air Force Reserve/Air National Guard outside the Altus local area, call us and we will coordinate your care.
Labs (HIV Testing)
HIV labs are walk-in services, no prior lab order required. However, lab orders must be entered into the system before the patient arrives.
