What We Do
The Primary Care Behavioral Health program integrates mental health services into primary care to provide comprehensive support for your well-being. Our team collaborates with your primary care manager to offer short-term assistance when habits, behaviors, or emotional concerns affect your daily life and health progress.
Services Offered

Consultation and Brief Intervention: We provide consultations and brief interventions, distinct from traditional psychotherapy. Our assessments and recommendations are documented in your medical record, and not in a separate mental health file.

Health Behavior Changes: We assist in developing and maintaining healthier lifestyle choices, addressing issues such as:
- Family or relationship difficulties
- Stress management
- Grief and loss
- Anxiety and worries
- Anger Management
- Depression

Clinical Health Problems: We support managing conditions like:
- Sleep/insomnia
- Pain management
- Migraine or tension headaches
- Fibromyalgia
- Diabetes
- Gastrointestinal issues or irritable bowel syndrome
- Smoking cessation
- Alcohol or substance use disorders
- Exercise and diet

Medication Compliance: We monitor patients diagnosed with anxiety, depression, PTSD, or other mental health conditions when a PCM has prescribed medication. During assessments, we educate about medication, treatment expectations, potential side effects, and possible remedies.
Eligibility and Access
PCBH services are available to all enrolled patients. Our team helps you build healthier habits, manage mental health concerns, and navigate emotional stressors without the need for a traditional mental health referral.
Easy Access to Care
- Same-day appointments are available as needed
- Self-referrals are welcome, no need to go through your primary care provider
